Executive Summary
The cost of hiring an employee is not limited to the salary agreed upon by the employee employer. The cost/benefit analysis becomes even more important when a rural public health agency is debating the decision to increase staff.

Hamilton County Public Health is a rural agency. The public health agency serves the entire county of Hamilton. The agency has an extended list of services. The agency has seen an increase in referrals for all the offered programs and is trying to determine if hiring another staff member would be beneficial to the agency. There are several factors that must be considered: The first external factor is the composition of the county being served, the second external factor that must be considered relates to the various funding sources of the agency and the third external factor that must be considered is the discussion of a possible merger with the local hospital.

As a rural public health agency, serving a primarily aging population, there are several major issue that must be addressed. The issues needing consideration: the cost of hiring a Registered Nurse, the cost of training/orientating a Registered Nurse, quality of care provided by agency, staff workload, funding availability to hire a new Registered Nurse. The discussion of hiring a Registered Nurse for Hamilton County Public Health must not only assist with meeting the issues addressed previously, as well as financially making sense, but also must assist with meeting measurable goals. The consideration of increasing staff must benefit the entire agency. By hiring a Registered Nurse, ultimately the goal for Hamilton County Public Health would be to assist in decreasing staff workload, increase staff retention, improve agency moral, increase staff satisfaction, improve agency reputation within the community and provide more continuity of care to client. Based on the findings of this report, it is recommended that Hamilton County Public Health move forward with the hiring of a new staff member.

Hamilton County Public Health is a local public health agency that serves a rural county in the state of Iowa. The agency is currently comprised of six Registered Nurses, two social workers, an administrative staff, Sanitarian and four direct care workers. Hamilton County Public Health is a Medicare/ Medicaid certified home care agency that has served all of Hamilton County since 1949. The agency offers an array of services to the residents of Hamilton County, such as: Home Visits by Skilled Nurses, PT, OT, Speech Therapy, Direct Care Staff, Elderly Case Management Services, Medical Social Work Services, Foot Clinics, Women, Infant and Children (WIC Program), HOPES Program (National Credentialed Healthy Opportunities for Parents to Experience Success), Maternal Infant Early Child Home Visits (MIECHV Program), Early Headstart, In Home Parenting Support, Maternal Child Health Clinics, Lead Poisoning Prevention, Immunizations, Respite Care for Adults, Well and Septic Permits, Rural Water Testing, Iowa Breast and Cervical cancer Early Detection (BCCEDP), Communicable Disease Follow-up, Community Health Education Programs, Bio-emergency Planning, Community Health Needs Assessment and Inspections of Pools, Spas, Tanning Beds and Tattoo Parlors.
